Requesting your
Health Care Records

Current and former clients have the right to request copies of their mental health records maintained by Be Counseling. Records will be provided in accordance with Texas law and abiding by applicable privacy regulations.

To request your records, please submit a written request to Be Counseling via email and include full name, description of records being requested and dates of treatment as well as preferred method to receive records.

Filing a Consumer Complaint

If you wish to file a consumer complaint regarding mental health services, you may do so with the Texas Office of the Attorney General: Office of the Attorney General - Consumer Protection.
